WHAT IF I NEED TO UPDATE OR REPLACE MY DOCUMENTATION?

If your current legal name, date of birth, or gender is different than what is displayed on your identity or lawful status document, you must show legal proof of the changes (i.e., marriage license). For example, a woman may need to obtain her marriage license to validate the change from the maiden name listed on her birth certificate. If a name change is necessary, citizens should visit the Social Security office. Request a replacement Social Security card if necessary. If you wish to obtain a certified copy of your birth certificate, contact the appropriate agency in the state of your birth. Those born in Kentucky may contact the Kentucky Office of Vital Statistics.
