Don't Forget!: If your current legal name, date of birth, or gender is different than what is displayed on your identity or lawful status document, you must show legal proof of the changes by bringing in additional documentation (i.e. marriage licenses, court orders, divorce papers).

Note: Proof of Residency documents may be in the name of the applicant's spouse or legal guardian. A marriage certificate or legal guardian form will need to be presented.
