Valid proof documentation is required to update the address or name on a credential.

Name Change Documentation Examples
- Certified Marriage Certificate (not marriage license)
- Certified Divorce Decree (must show that a name change was granted)
- Valid Military ID
- Certified Legal Name Change Order
Update by Mail
Proof of the address or name change must be attached to the application. Do not send original documents.
Update in Person
Visit any Driver Licensing Regional Office. Proof of the address or name change must be presented.
Update Requirements
Kentucky law requires you to update your name or address within 10 days of the change. A duplicate credential costs
$15.00 and can be requested by mail or in person.
In addition to providing the name change documentation, you must provide proof that the name has been updated with the Social Security Administration. A SSNAP form is accepted if it has been at least 48 hours since approval.
