​Applying for Kentucky Transportation Cabinet Vision Screening Credentials​:

  • Vision specialists are not required to obtain certification from the Kentucky Transportation Cabinet (KYTC) to perform vision screenings for driver's license renewals.

​*​The final version of the Medical Specialist Credentialing Certification (TC 94-203) has been sent to licensure boards for medical professionals interested in credentialing​. ​

Failed Screenings: Drivers who do not pass their vision screening at a Drivers Licensing Regional Office or with a KYTC-credentialed medical professional will be referred to a vision specialist for a secondary vision examination and any corrective lenses. Vision specialists may then fill out and sign a TC 94-202 Driver Vision Testing Certification form, confirming their patient can safely operate a vehicle and proceed with license renewal.​


Corrective Eyewear: Applicants may perform the screening wearing corrective eyewear, but that must be noted on the form if completed by a medical professional. The patient's license will stipulate that lenses must be worn when operating a vehicle.


Online/Mail-in Renewal: Kentuckians may still renew their driver's license online or by mail. The online renewal portal will allow users to upload a screening form signed by a vision specialist or KYTC-credentialed medical professional.

UNDERSTANDING THE TC 94-202 DRIVER VISION TESTING CERTIFICATION FORM​

​The TC 94-202 form is a critical component of Kentucky's driver vision testing process. This document ensures that all vision screenings conducted outside a Kentucky Driver Licensing Regional Office are properly certified and meet state requirements. 

​A. ​​When is the TC 94-202 Form Required?

  • ​​If a vision test is performed by a vision specialist or a KYTC-credentialed medical professional outside of a Kentucky Driver Licensing Regional Office, the TC 94-202 form is required as proof of compliance.

  • This form is the only accepted documentation to verify the successful completion of a vision screening under these circumstances.​

  • Vision specialists requiring the TC 94-202 Driver Vision Testing Certification Form can request it by emailing KYTC at kytc.ddlvisionspec@ky.gov​.
​​​​B. When is the TC 94-202 Form NOT Required?
  • ​​Drivers who undergo and pass vision screenings at Kentucky Driver Licensing Regional Offices do not need to submit the TC 94-202 form​. Results are automatically recorded in the system.

  • Drivers who do not pass their vision screening at a Drivers Licensing Regional Office will be referred to a vision specialist for secondary vision examination and any corrective lenses.​

    Vision specialists may then fill out and sign a TC 94-202 Driver Vision Testing Certification form, confirming their patient can safely operate a vehicle and proceed with license renewal.​ The driver can then bring the form to any Regional Office, upload it through the online license renewal portal, or include it with a mail-in renewal application.​
