Disabled Parking Permits

​Applications for permits are available at any County Clerk’s office, or the application can be printed from the following link: TC 96-347 APPLICATION FOR DISABLED LICENSE PLATE OR PARKING PLACARD. Kentucky residents can apply for their parking placard in person or mail their completed application to their local County Clerk's office.

  • ​Persons whose mobility, flexibility, coordination, respiration, or perceptiveness is significantly reduced by a permanent or temporary disability to the arms, legs, lungs, heart, ears, or eyes may obtain a permit with a licensed physician, chiropractor, or advanced practice registered nurse's original signature on the application for the permit.

  • Current and new (permanent) placard applicants will be required to complete the Application for Disabled License Plate or Parking Placard Form (TC 96-347) and have the form signed by a licensed physician, chiropractor, or advanced practice registered nurse's original signature. This multi-use form allows applications for temporary and permanent disabled placards as well as disabled license plates.

  • For an applicant who is obtaining a temporary placard certification, the Application for Disabled Parking Placard Form (TC 96-347) may be completed by a licensed physician, physician assistant, physical therapist, occupational therapist, chiropractor, or advanced practice registered nurse's original signature.

  • The applicant's signature must be notarized. Applicants can have their signature notarized at any County Clerk's office. The notary fee is $2.00.

  • Permanent disabled placards are valid for 6 years and may be renewed an unlimited number of times. The first permanent placard (blue in color) is free, while a second or replacement placard costs $10.00.

  • Temporary disabled placards (red in color) are valid for a 3-month period. No fee is charged upon issuance; replacement placards are $ 10.00.

  • If either placard type sustains damage, the County Clerk will issue a replacement placard at no cost.

  • Both permanent and temporary placards shall be displayed on the front windshield's rearview mirror. The placards shall be displayed on the dashboard when there is no rearview mirror.
